Create a conceptual model and strategic plan for spreading the transformation of primary care practices into patient-centered medical homes via physician organizations and similar groups based on results from the following:
1) Review of literature on spread theory and its application in ambulatory healthcare settings
2) Needs assessment and analysis of the current capacity of physician organizations and physician-hospital organizations in Michigan to support primary care practices in transformation activities
3) Analysis of the successful strategies used in at least three U.S. demonstration projects
4) Identification of public and private sources of financing for the practice transformation process
