MPCC members are involved in various activities to achieve the overall objective:
Comprehensive, coordinated, whole-person care that is adequately reimbursed will be provided by every primary care setting in Michigan
MPCC facilitated preparation of the successful application to CMS by the Michigan Department of Community Health. In November, 2011, Michigan was selected as one of eight states with Patient-Centered Medical Home pilots that CMS will join for a three year demonstration project. Enrollment of practices in the Michigan Primary Care Transformation Project is in progress. The Demonstration will begin January 1, 2011 and go for three years. For more information click here
Michigan payers agreed that beginning in 2010, they would include three specific PCMH components in their incentive programs:
1. Extended access
2. E-prescribe
3. Registry use
Additional measures added for 2011 are
1. ED use
2. Registry functionality
a. Has decision support rules
b. Can do performance reporting for at least one clinical measure.
Each health plan applies the metrics through its own contracting and incentive programs.
