As hubs for the communication of essential health information and coordination of care, patient centered medical homes are an integral part of communities and the larger healthcare system. Healthcare networks and systems can be organized to drive high quality, cost effective care.

  • Accountable Care Organizations: ACOs put strong financial incentives in place to reward coordination of care across specialties and settings. Such organizations as the Mayo Clinic, the Geisinger Health System, Kaiser Permanente and the Cleveland Clinic have demonstrated improved care and reduced costs.
